Club Survey

This survey is designed to be completed by authorised representatives of sporting clubs that play any of the sports listed in the project scope. This survey will take between 5 and 10 minutes to complete and requires you to:

• Have a copy of your club's strategic plan (if applicable); and
• Provide membership/playing data (if known).
